- Fixed a bug in which a PC/SC session was not being terminated if Test Mode was canceled from the 
  Password dialog (i.e., password was not saved when starting Test mode). This required restarting
  SmartCache to perform further operations with a PC/SC reader.
- Fixed a bug in which the Hide checkboxes did not function if a file was loaded before a successful
  Read operation was performed.
- Updated setup of the Initialization dialog. If the card password is saved, pressing the Initialize
  button now opens the Initialization dialog with the password fields initialized. This
  is very useful for emulation testing.
